1. Clinical Vignette
I want to make sure that you all are understanding the concept of just “plugging” in information that the full assessment is asking. (again this form is under the modules tab and will need to be used for Clinical vignettes #1, 3-5). I would like to see each section expressed in your paper clearly and in paragraph format.
The only sections you do not need to write about in the vignette is medical history, substance abuse, mental status exam. Please don’t forget in the last section of your paper which is the case formulation to summarize everything here, clients symptoms, What types of treatments would you do if you were his therapist? What is the diagnosis. Be able to support this with evidence in your findings (DSM). Clinical Vignette Myles is a 20 year-old Caucasian man who was brought to the emergency room by the campus police at USC from which he had been suspended several months ago. A professor had called and reported that Myles had walked into his classroom, accused him of taking his tuition money and refused to leave.
Although Myles had much academic success as a teenager, his behavior had become increasingly odd during the past year. He quit seeing his friends and no longer seemed to care about his appearance or social pursuits. He began wearing the same clothes each day and seldom bathed. He lived with several family members but rarely spoke to any of them. When he did talk to them, he said he had found clues that his college was just a front for an organized crime operation. He had been suspended from college because of missing many classes. His sister said that she had often seen him mumbling quietly to himself and at times he seemed to be talking to people who were not there. He would emerge from his room and ask his family to be quiet even when they were not making any noise.
Myles began talking about organized crime so often that his father and sister brought him to the emergency room. On exam there, Myles was found to be a poorly groomed young man who seemed inattentive and preoccupied. His family said that they had never known him to use drugs or alcohol, and his drug screening results were negative. He did not want to eat the meal offered by the hospital staff and voiced concern that they might be trying to hide drugs in his food.
His father and sister told the staff that Myles’ great-grandmother had had a serious illness and had lived for 30 years in a state hospital, which they believed was a mental hospital. Myles’ mother left the family when Myles was very young. She has been out of touch with them, and they thought she might have been treated for mental problems. Myles agreed to sign himself into the psychiatric unit for treatment.
2. Discussion
Sometimes individuals who are severely mentally ill are forced into treatment. For a total of 25 points, participate in this discussion board on the topic. First, respond to the following question and try to examine it from both sides of the issue (i.e., try to articulate the argument from the perspective of both “yes” and “no”):
Is forced treatment of seriously mentally ill individuals justifiable? In other words, do you think people should be treated against their own stated wishes? Why or why not? Again, be sure to examine this issue from both sides (i.e., demonstrate that you can see multiple perspectives).
Type your response (5-10 sentences), but remember your typed response must be free of typos and grammatical errors for full credit (15 points). For the remaining 10 points, respond constructively to one of your classmates’ posts. Use this opportunity to help one other learn and apply the material. 3. Discussion 2
1. Briefly summarize the facts of the Tarasoff case and its primary implication for clinical psychologists. (Have at least 3-5 sentences)
2. Briefly describe the challenges related to confidentiality when the psychotherapy client is a child or adolescent, as well as suggestions for resolving these challenges. (Have at least 3-5 sentences)
